Application Forms for Registration for Professional Soil Classifier in Training

There are four options with the online fill-in forms depending on the type of software you have. 1.) If you have Adobe Acrobat Reader software (free download at: http://www.adobe.com/prodindex/acrobat/ ), you may save and/or print the forms for hand completion and hardcopy submission via snail-mail to the Board’s mailing address: North Dakota Board of Registration for Professional Soil Classifiers, P.O. Box 5638 University Station, Fargo, ND 58105. 2.) Adobe Acrobat Reader can be used to fill-in the forms but the completed form cannot be saved to a pdf file. However, the completed form can be printed out and submitted via snail-mail. 3.) If you have Adobe Acrobat Reader version 6 or greater you can export the fill-in data to a file and save. That file can be submitted to the Board via e-mail mike.ulmer@msn.com. 4) If you have Adobe Acrobat software you can fill-in and save the completed form as a pdf file and submit to the Board via e-mail.
